Guilt sucks. It really does. And it's hard to shake, for it's not just about forgiving ourselves. That much is easy. But what are to do when we feel we have disappointed others? A certain code, God, Mommy and Daddy... the list is endless.
Here is ONE option, an affirmation:
"I accept disappointing X"
But if people would recap all their lives, this wouldn't be an issue. Alas, we need to be strong enough to do that first.
I recommend to first deal with God-guilt issues first, for it is the most corrosive form of guilt there is. You are cutting yourself from the very source, with that one.
